Fix some random stuff in QueuedOutputStream

* Fix the destroy notify for the GAsyncQueue * Add some comments around some hard to follow stuff * Add a missing transfer annotation.
Ran the unit tests undergrind and also ran the xeme unit tests under valgrind against this code.
